The Blood Twins can be found not far beyond Itsy's lair. When you first enter their lair, you will cross through blood red water in which contains Lurker style monsters called abnormalities that can occasionally drop Stat Pots
There is a Female blood twin and a Male blood twin.
The female can only be killed by magic/blue glow weapons and she will attack all characters in sight. Best strategy is to keep 3 hex dark up while she is alive.

The twins are known to drop a scroll (worth a lot of money), fan, shuriken (which can be upgraded in Leng), and rarely, a set of MA armor.
